The Outcome Based Treatment Plan module is located in the Client Chart making the treatment plan itself the focus of the medical record. Assessments, lab, and vital screenings, in-therapy discussions, and progress notes now feed into the Client Face Sheet making it easy to address problems as they arise, view progress, document successes or areas of needed improvement, and update changes that ultimately result in positive, life-changing outcomes for the Client. Using the same information that feeds the treatment plan, encounters are created which in turn guide the service entry process, allowing Staff to fully focus their attention on the client.

Configuration Overview

• User Group Menu Access to Configuration > Client Chart > Treatment Plans is needed for the users responsible for configuring the following treatment plan items.

  • Objectives

  • Milestones

  • Problems

  • Stages of Change

• User Group Access to the Face Sheet in the Clients main menu option is needed for the users responsible for creating, reviewing, updating, and closing a client’s treatment plan.

• The Defined List for Objective Completion Reasons should be populated with the agency-specific completion reasons in Configuration > Setup > Defined Lists. These values populated the drop-down list when a user marks an objective complete.

• The database schema for treatment plans from the Client Chart is txp.

• Treatment plan summary and problem forms are FDNext forms. Please contact Echo for changes to existing forms or to add a new summary or problem form.
