The PHQ-9 Modified for Adolescents also known as a PHQ-A, is the second of the two assessments included standard with EchoVantage.  This is assessment is a tool to measure the severity of depression. The form used is the PHQ-9 Modified for Adolescents. Form responses, including total score, are stored in the dbo.PHQ9ModifiedAdolescents table. This 13 question assessment has the same navigation and functionality as the PHQ9 assessment.  A sample PHQ-9 Adolescent assessment form is displayed below.
