The BH Demographic form is designed to capture Social and Behavioral Domains and Measures in a format and frequency that satisfies Meaningful Use criteria. This form combined with Client Profile and Communications tab information addresses the four core and eight additional domains identified for Stage 2 and 3 compliance. 

The Behavioral Health Demographic form responses are stored in the dbo.BHDemographic table. There is no history form associated. From the client Timeline, Click on the green plus "+" next to the form name to add responses to a New form.

The form opens to the first tab, Demographic, by default. This tab covers the Educational Attainment, Financial Resources Strain, and Physical Activity domain requirements.

The Demographic Cont. section allows the collection of gender and sexual orientation when provided by the client per MU Phase 3 requirements/recommendations.

The Social Connection and Isolation Panel is the NHANES assessment. As responses are entered, the Total Social Isolation Score displayed at the end of the assessment is updated.

The Hark Screening is used to assess Intimate Partner Violence and is recommended for all women of child bearing age. This screen will updated the Total Score or HARK at the bottom as responses are entered.

The Audit-C is a short 3-item screen that can help identify clients who are hazardous drinkers or have active alcohol use disorders. The Total Score updates as responses are recorded.

The PHQ-2 is included for a first-step approach to depression screening. Clients that screen positive would most likely be further evaluated with a full assessment such as a PHQ-9.  Again the Total Score updates as responses are entered. 

As the BH Demographic screens are completed, a depiction will display on the Timeline. The graphic displayed can be set by a database administrator in Configuration>Setup>VHR Timeline.
