The Family Health History form allows information such as Relation and Diagnoses of the client's family members to be stored together. The last section of the form keeps track of when and by who the information of the form is reviewed. The Diagnosis drop-down list is supplied by Echo and is a view found in dbo.VFHHDiagnoses. This view is based upon OID 2.16.840.1.113883.3.88.12.3221.7.4 (Problem Value Set) and is the same SNOMED list available on the Client Diagnosis screen. 

The SEND button (envelope icon) can be used to send a review request to another EchoVantage user. When Review Requested is selected, an icon is available that links back to this specific form.
